본 연구에서는 3D 프린팅 기술과 인체공학 순설계를 이용하여 턱관절 자기공명영상 동적 턱관절 검사 보조기구를 개발하고자 하였다. 3D 프린팅 기술 재료는 3D 프린터(sindo, 3DWOX1, Korea), 3D 모델러 프로그램(Fusion 360, autodesk, USA), PLA (PolyLactic Acid) 필라멘트 소재를 이용하였고, 영상검사는 3.0T 자기공명영상 장비(Magnetom Vida, Siemens, Germany)와 두경부 코일(Head&Neck coil)을 사용하였다. 개발방법은 성인 30명(남:13명, 여:17명, 평균나이 22.9±2.0세)과 소아 30명(남:21명, 여:9명, 평균나이 11.6±1세)의 안면뼈 CT (Computed Tomography) 검사의 단면영상을 역학적으로 실측하여 상/하악궁의 형상을 모델링하였다. 보조기구의 동적 개구 기능은 기어방식으로 0.1 cm 씩 이동할 수 있도록 하고, 두경부 코일 내에서 작동할 수 있도록 순설계하였다. 모델링된 파일은 STL로 변환하고 G-code를 생성 후 오류여부를 확인하고 FDM (Fused deposition modeling) 방식으로 3D 프린팅하였다. 출력된 보조기구는 자화 감수성 인공물 실험, 동적 영상 비교, 만족도 평가로 성능을 평가하였다. 자화 감수성 인공물 실험은 PD (Proton Density) 의 단면영상에서 개발된 개구 보조장비와 SNR (Signal to noise)을 비교하였다.기존 보조장비와의 비교는 PD 시상면 영상에서 비교하였다. 동적 영상 비교는 CINE 기법, FLASH (Fast low angle shot) 기법, TSE (Turbo spinecho) 기법을 SNR과 SSP (section sensitivity profile)로 정량적 평가를 실시하였고, 전문의의 정성적평가로 실시 만족도평가는 피검자 7명와 방사선사 7명을 대상으로 리커트 5점 척도로 평가하였다. 그 결과, 자화감수성 인공물 발생은 개발된 개구 보조장비와 비교하여 모든 영상에서 유의한 차이가 없었다. 동적 비교 영상에서는 TSE 기법이 모든 평가법에서 가장 우수한 영상품질을 보였다. 만족도 평가에서는 피검자는 평균 4.3점, 방사선사는 평균 4.4점으로 높은 만족도를 보였다. 결론적으로 인체공학적으로 설계된 환자 맞춤형 보조기구에 개구의 동적 기능이 탑재된 보조기구를 개발하였다. 개발된 보조기구는 검사의 편리성을 제공할 수 있으며, TSE 기법으로 턱관절 움직임에 따른 디스크 변형의 정밀한 관찰 및 진단에 유용할 것으로 기대한다.
This study attempted to develop a temporomandibular joint magnetic resonance imaging dynamic temporomandibular joint study device using 3D printing technology and ergonomic design. The 3D printing technology materials used 3D printers (Sindo, 3DWOX1, Korea), 3D modeler programs (Fusion 360, Autodesk, USA), and PLA (PolyLactic Acid) filament materials, while 3.0T magnetic resonance imaging equipment (Magnetom Vida, Siemens, Germany) and head & neck coil were used for imaging tests. The development method modeled the shape of the upper and lower temporomandibular arch by mechanically measuring the cross-sectional images of facial bone CT (Computed tomography) of 30 adults (male:13, female:17, average age of 22.9±2.0 years) and 30 children (male:21, female:9, average age of 11.6±1 years). The dynamic opening function of the assistive device is designed to move by 0.1 cm in the gear method and to operate within the head & neck coil. The modeled file was converted to STL, G-code was created, checked for errors, and 3D printed using FDM (Fused deposition modeling). The output assistive device evaluated its performance by experimenting with susceptibility artifacts, comparing dynamic images, and evaluating satisfaction. The susceptibility artifact experiment compared the SNR (Signal to noise) with the opening device developed in the cross-sectional image of the PD (Proton density). The comparison with the conventional device was compared in the PD sagittal plane image. For dynamic image comparison, the CINE technique, FLASH (Fast low angle shot) technique, and TSE (Turbo spin echo) technique were quantitatively evaluated with SNR and SSP (Section sensitivity profile). Satisfaction evaluation was evaluated on a 5-point Likert scale for 7 subjects and 7 radiological technologists. As a result, there was no significant difference in the occurrence of susceptibility artifacts in all images compared to developed opening devices. In the dynamic comparison image, the TSE technique showed the best image quality in all evaluation methods. In the satisfaction evaluation, subjects averaged 4.3 points and radiological technologists averaged 4.4 points, showing high satisfaction. In conclusion, we developed a device equipped with the dynamic function of the opening in an ergonomically designed patient-specific device. The developed device can provide convenience for examination and is expected to be useful for precise observation and diagnosis of disc deformation according to temporomandibular joint movement with the TSE technique.
