Objective: Improving health levels is a major challenge for the nation's competitiveness by enhancing the quality of human capital as well as the fundamental rights of the constitution-guaranteed health rights. To this end, the government emphasized the importance of health investment and promoted various policies to change the paradigm as a preventive health policy. When implementing health policies, we would like to propose effective policy implementation tasks to realize the vision of health in all policies. Method: Through a survey of international health status and Korean health policies over the past 10 years, we analyzed how health investment and preventive health promotion concepts are reflected in health policies. In the analysis, the policy direction was reviewed by comparing it with Korea in three aspects: socioeconomic factors, policy enforcement factors, and policy resource factors. Results: According to the International health policy analysis, health policies have shifted the paradigm from cure to prevention or management for preventive health care, securing quality human capital and increasing national competitiveness by investing in health. To achieve this, national policies are driven around health equity based on health (HiAP) and the policy foundation has been established for maintaining and realizing health policies through pan-governmental framework, as well as health-related departments. Through this, as the last task to establish a framework for social security with health investment through preventive ministry treatment, the government should contribute to the actual implementation of health rights guaranteed under the Constitution of Prevention to Treatment by setting health agenda as "the role of the nation" instead of leaving it to "individual areas." Conclusion: Although long-term strategies were prepared through the establishment of the National Health Plan 2030 in Korea, specific strategies for realizing them are insufficient. To this end, the vision and major tasks of the policy are to promote government-wide governance and health investment for health in all policies, and to conduct health impact assessments for mainstreaming and effectiveness of health policies.
연구목적: 건강수준의 향상은 헌법에 보장된 건강권으로 기본권적인 측면뿐 아니라 인적 자본의 질적제고를 통하여 국가의 경쟁력을 상승시킬 수 있는 주요한 과제이다. 이를 위하여 정부에서는 건강투자에 대한 중요성을 강조하고 사전 예방적인 건강증진 정책으로서 패러다임을 전환하고자 다양한 정책을 추진하였다. 건강정책 추진시 모든 정책에 건강이라는 비전을 실현하기 위한 실효성 있는 정책 추진방향과 실행과제를 제언하고자 한다. 연구방법: 지난 10년간의 국제보건현황과 국내외 건강정책 현황 조사를 통하여 건강투자와 사전 예방적 건강증진 개념이 건강정책에 어떻게 반영되고 있는지 분석하였다. 분석시에는 사회경제적 요인, 정책집행요인, 정책집행자원요인 3가지 측면에서 우리나라와 비교분석하여 정책방향을 검토하였다. 주요결과: WHO와 국외 건강정책현황 조사 분석 결과, 건강투자가 질 높은 인적자본 확보, 국가 경쟁력 상승의 중요한 요소임을 강조하였으며, 국내외 건강정책은 사전 예방적인 건강관리를 위하여 치료에서 예방 또는 관리로 패러다임을 전환하였다. 이를 실현하기 위하여 국가별 정책은 모든 정책에 건강을(HiAP)를 기반으로 건강형평성 제고를 중심으로 추진하고 있으며, 정책기반으로 보건과 관련된 부서뿐만 아니라 다른 분야의 부서까지 포함된 범정부적 거버넌스 구축, 건강정책 주류화 등을 마련하였다결론: 우리나라는 국민건강증진종합계획 2030을 수립하여 중장기 전략을 마련하였으나, 이에 대한 구체적인 실현전략은 미흡하다. 이를 위하여 정책의 비전과 주요과제는 모든 정책에 건강을 위한 범정부 차원의 거버넌스 구성과 건강투자를 추진하고, 건강정책의 주류화 및 실효성을 위한 건강영향평가를 실시하는 것이 필요하다. 이를 통하여 예방부터 치료까지 건강투자를 통한 사전 예방적인 건강정책으로 사회보장 기본틀 세우기에 기여하며, 건강을 ‘개인의 영역’에 맡기지 않고 ‘국가의 역할’로 역할을 부여함으로 ‘예방부터 치료까지’라는 헌법에서 보장된 실질적 건강권 구현에 기여해야 하겠다.
