Symptoms of lumbar spinal stenosis may not be present even if the cross-sectional area of the dural tube appears narrow on magnetic resonance imaging (MRI). It is important to achieve diagnosis not only in accordance with MRI findings, but also in accordance with neurological findings. In lumbar spinal stenosis, neurological findings may be absent at rest since intermittent claudication is induced by walking. However, diagnostic accuracy can be improved using a gait-loading test. Detailed pathological evaluation and strict operative indications and selection of a suitable operative procedure are indispensable for optimizing the operative results of lumbar spinal stenosis.
