This paper proposes a metasurface design method for antenna beam-forming. Based on tangential electromagnetic fields on both sides of the metasurface, the equivalent model of the metasurface—impedance sheet model(ISM) is calculated. Accordingly, the metasurface structure is designed. The given examples illustrate that ISM and metasurface can transform antenna pattern into target field pattern and realize beam-forming.
