目的 探讨精神分裂症患者其暴力攻击行为与事件相关电位P300 成分的相关性.方法 将 2020 年 12 月—2022 年12 月福建省福州神经精神病防治院精神科精神分裂症住院患者根据其既往个人史及修改版外显攻击量表(modified overt aggression scales,MOAS)评分划分暴力攻击组 30 例及对照组 31 例.对比 2 组一般资料、P300 成分的波幅和潜伏期;以及P300成分的波幅和潜伏期与MOAS评分的相关性分析.结果 精神分裂症患者中暴力攻击组与对照组一般资料差异无统计学意义(P>0.05);但所诱发的P300 成分波幅和潜伏期差异有统计学意义(P<0.05),其中,暴力攻击组其P300 成分波幅降低(2.818±0.643)μV,潜伏期延长(335.530±20.664)ms.经Pearson相关分析,精神分裂症患者所诱发的P300 成分潜伏期与MOAS评分呈正相关(r= 0.580,P<0.05),而其所诱发的P300 成分波幅与MOAS评分呈负相关(r=-0.421,P<0.05).结论 具有暴力攻击行为的精神分裂症患者存在更严重的脑认知功能损伤.本研究推测,事件相关电位P300 成分的波幅和潜伏期可作为预测及评估精神分裂症暴力攻击行为的电生理指标.
Objective To investigate the correlation between aggressive behavior and event-related potential P300 in patients with schizophrenia.Methods From December 2020 to December 2022,all patients with schizophrenia in department of psychiatry,Fujian Provincial Fuzhou Neuropsychiatric Prevention and Treatment Hospital divided into the violent aggression group with 30 cases and the control group with 31 cases according to the previous personal history and the score of the modified overt aggression scales(MOAS).The general data,the amplitude and latency of P300 components of two groups were compared.Moreover,correlations were calculated between the amplitude and latency of P300 components and MOAS scores of all patients.Results There were no significant differences between the general data of the violent aggression group and the control group(P>0.05),but the amplitude and latency of P300 components were statistically significant(P<0.05).The P300 amplitude decreased significantly(2.818±0.643)μV and the P300 latency prolonged(335.530±20.664)ms significantly in the violent aggression group.The Pearson's correlation analysis demonstrated the patients'latency of P300 was positively correlated with their MOAS score(r=0.580,P<0.05),while their P300 amplitude was negatively correlated with their MOAS score(r=-0.421,P<0.05).Conclusion Schizophrenic patients with violent aggression had more severe cognitive impairment.This study speculated that the amplitude and latency of the P300 component can act as a electrophysiological index to predict and assess schizophrenic patients'violent behaviors.
