目的:探讨肾性骨营养不良的CT表现,提高对肾性骨营养不良的认识和诊断能力。方法收集2011年1月至2015年5月我院临床和影像资料完整的58例肾性骨营养不良病例,螺旋扫描后薄层多平面重建,多窗位观察,分析受累骨形态、骨皮质、骨小梁的形态和密度改变特点,以及软组织内的异常钙化。结果骨质改变表现为骨皮质增厚,颅底骨质呈象牙质样改变;髓腔内可见小囊样改变,囊壁模糊,骨小梁边缘模糊;椎体可呈楔形改变;皮下软组织内见点状钙化,眶内小血管壁钙化;软组织内的钙化常两侧对称。结论肾性骨营养不良的CT改变有一定特点,结合临床表现及病史能作出正确诊断。
Objective To analyze and discuss the CT manifestations of renal osteodystrophy, and to improve the recognition and diagnosis of it. Methods Clinical and imaging data of 58 cases of renal osteodystrophy, who admit-ted to our hospital from January 2011 to May 2015, were collected. After spiral CT scan, the thin layer multi-planar was reconstructed and observation was performed through multi-window. The morphology and density changes of the bone, cortical bone and trabecular bone as well as abnormal calcifications in soft tissue were analyzed. Results The CT mani-festations of renal osteodystrophy were characterized by thickening of cortical bone, ivory like changes of skull base, vis-ible intramedullary follicle-like changes, fuzzy cystic walls and fuzzy edges of trabecular bone, wedge-shaped changes of centrum, punctate calcifications in subcutaneous soft tissue and calcifications in the wall of orbital small vessels. Cal-cifications in the soft tissue were usually bilaterally symmetrical. Conclusion CT image changes in renal bone dystro-phy have certain characteristics. Clinical manifestations combined with medical history can facilitate correct diagnosis of renal osteodystrophy.
