目的:探讨不同制备时间对去白细胞悬浮红细胞质量的影响.方法:选取南平市中心血站2023年5月21日采集的同一批次160份献血样本,随机分为研究组与对照组,各80份.对照组于采集血液后置4 ℃冰箱储存,待血液温度降到10 ℃左右时进行白细胞过滤并离心,制备成去白细胞悬浮红细胞和血浆.研究组于血液采集后置4 ℃冰箱储存,待血液温度降到10 ℃左右时进行白细胞过滤,但不离心,根据储存时间不同,将过滤后血液分成四个亚组:研究1组(20份,储存1 d),研究2组(20份,储存5 d)、研究3组(20份,储存9 d),研究4组(20份,储存14 d),于储存期末取出,离心分离制备成去白细胞悬浮红细胞和普通血浆,比较储存期末溶血率、血浆游离血红蛋白浓度、血常规指标、血液流变学指标差异.结果:不同组别样本的储存期末红细胞溶血率比较,差异均无统计学意义(P>0.05).研究组样本的血浆游离血红蛋白浓度明显高于对照组,且随着制备时间的延长,血浆游离血红蛋白浓度也显著增加,差异均具有统计学意义(P<0.001).不同组别样本的白细胞残留量、去白悬浮红细胞容量、红细胞比容比较,差异均无统计学意义(P>0.05).研究组样本的红细胞聚集指数、红细胞刚性指数、红细胞电泳指数都显著高于对照组,差异均具有统计学意义(P<0.001),且随着制备时间的延长,红细胞聚集指数、红细胞刚性指数、红细胞电泳指数也显著增加,差异均具有统计学意义(P<0.001);不同组别的红细胞变性指数比较,差异均无统计学意义(P>0.05).结论:去白细胞悬浮红细胞的制备时间延长虽然不会增加储存期末红细胞溶血率,但血浆游离血红蛋白浓度增加,红细胞聚集指数、红细胞刚性指数、红细胞电泳指数升高,因此需严格控制去白细胞悬浮红细胞的制备时间.
Objective To investigate and analyze the effect of different preparation times on the quality of leukocyte-reduced red blood cell suspension.Methods A total of 160 blood donation samples from the same batch collected on May 21,2023 in Nanping Central Blood Station were randomly divided into a study group and a control group,with 80 samples in each group.The control group was stored in a refrigerator at 4 ℃ after blood collection.When the blood temperature dropped to about 10 ℃,white blood cells were filtered and centrifuged to prepare leukocyte-reduced red blood cell suspension and plasma.The study group stored the blood in a refrigerator at 4 ℃ after blood collection.When the blood temperature dropped to about 10 ℃,white blood cells were filtered,but not centrifuged.According to different storage time,the filtered blood was divided into four groups:study group 1(20 samples,stored for 1 day),study group 2(20 samples,stored for 5 days),study group 3(20 samples,stored for 9 days),study group 4(20 samples,stored for 14 days)were removed at the end of storage,and the leukocyte-reduced red blood cell suspension and plasma were centrifuged.The differences of hemolysis rate,plasma free hemoglobin concentration,blood routine index and hemorheology index were compared at the end of blood storage.Results There was no significant difference in red blood cell hemolysis rate between the study group and the control group(P>0.05).The plasma free hemoglobin concentration in the study group was significantly higher than that in the control group,and with the extension of preparation time,the plasma free hemoglobin concentration was also significantly increased,and the differences were statistically significant(P<0.001).There were no significant differences in the residual amount of white blood cells,the volume of white removed suspension red blood cells and hematocrit among different groups(P>0.05).The erythrocyte aggregation index,erythrocyte rigidity index and erythrocyte electrophoresis index of the study group were significantly higher than those of the control group(P<0.001),and with the extension of preparation time,erythrocyte aggregation index,erythrocyte rigidity index and erythrocyte electrophoresis index were also significantly increased,and there were significant differences between groups(P<0.001).There was no significant difference in the erythrocyte degeneration index comparison among the different groups(P>0.05).Conclusion The extension of preparation time of leukocyte-reduced red blood cell suspension can lead to the increase of free hemoglobin concentration,resulting in the increase of erythrocyte aggregation index,erythrocyte rigidity index and erythrocyte electrophoresis index,so the preparation time of leukocyte-reduced red blood cell suspension should be strictly controlled.
