目的 分析不同病情程度反流性食管炎(RE)患者食管下段黏膜标本病原菌学与指导意义.方法 选取我院2021年9月至2022年3月期间收治的RE患者112例作为研究对象,依据RE洛杉矶分型可分成轻度组74例(A级+B级)、重度组38例(C级+D级),另选取同期健康体检者120例作为对照组,提取全部研究对象的食管下段黏膜标本细菌基因组DNA,通过16SrDNA基因扩增与Illumina测序技术归类细菌.结果 三组于γ-变形杆菌纲占比比较,差异存在统计学意义,且随病情加剧,γ-变形菌纲的占比就越高(P<0.05);三组普食菌属、螺杆菌属、莫氏杆菌属对比,差异存在统计学意义,对照组高于轻度组与重度组(P<0.05).结论 RE患者食管下段黏膜标本中病原菌构成产生显著改变,可能为临床防治提供方向.
Objective To analyze the pathogenic bacteriology and guiding significance of lower esophageal mucosa sam-ples in patients with different degrees of reflux esophagitis(RE).Methods A total of 112 patients with RE admitted to our hospital from September 2021 to March 2022 were selected as the research subjects.According to the Los Angeles classifica-tion of RE,they could be divided into mild group(74 cases)(A grade+B grade)and 38 severe group(C grade)Another 120healthy subjects were selected as the control group during the same period.Bacterial genomic DNA was extracted from the lower esophageal mucosa samples of all subjects,and the bacteria were classified by 16S rDNA gene amplification and Illumina sequencing technology.Results There were statistically significant differences among the three groups in the ge-nus Commonweed,Helicobacter,and Morinella.The proportions of γ-Proteobacteria among the three groups in the control group showed a statistically significant difference,and with the aggravation of the disease,γγ-the higher the proportion of Pro-teobacteria(P<0.05);Significantly higher than the mild group and severe group(P<0.05).Conclusion The composi-tion of pathogenic bacteria in the lower esophageal mucosa samples of RE patients changes significantly,which may provide directions for clinical prevention and treatment.
