INTRODUCTION The nucleolus is responsible for ribosome biogenesis. In recent years, our knowledge of the human nucleolar function has been expanded. Recent reports have elucidated its involvement in others important functions, like tumour suppression and oncogenesis. Nucleolar size has been correlated to proliferation rate of cancer cells tissue, with a potential prognostic role of this parameter. The present study has been undertaken with the aim of quantitatively investigate the value of nucleoli in the prognosis of clear cell renal cell carcinoma (CCRCC). MATERIALS AND METHODS 92 patients with renal cell carcinoma (RCC) surgically treated with adequate follow-up data and tumoral tissue for ultrastructural examination, were included in this retrospective study. Nucleolar areas were obtained by electron microscopy. The mean of nucleolar areas (MNA) and maximum nucleolar area (MaxNA) were calculated by ultrastructural morphometry study. Fuhrman’s tumor grade, 2004 TNM system, tumoral size, clinical status and the length of survival were recorded in each case. The results were assessed by univariate and multivariate analysis. RESULTS Significant differences were found between nuclear grade and both progression-free interval and survival. There were statistically significant differences between all TNM stages in progression-free interval and survival. TNM stage showed a significant relation-ship with MNA (Rho=0.23, p
