Effective patient education improves patient knowledge, engagement in their health, and long-term health outcomes. Health education using technology is an important step forward in teaching and learning. Current teaching styles must change to align with current learning styles. The primary objective was to develop and validate a patient-oriented innovative online education program for warfarin therapy. Secondary objectives included assessment of patient / family stress around learning, patient preference for on-line learning style, and health care provider (HCP) teaching time. This multi-center prospective cohort of children and young adults (median age 4 years) initiating warfarin (July 2017-April 2019) evaluates eKITE (kidclot.com) knowledge transfer using validated quizzes and surveys (patient preference, stress around learning STAI-1). Health care provider teaching time was measured and compared. Time in therapeutic range was calculated as a surrogate for safe and effective warfarin management. INR meters and test strips were provided when available. Six US and CDN sites enrolled 83 patients. Indications for anticoagulation were cardiac 96% (Fontan, artificial heart valves, intra-cardiac devices, Kawasaki?s) and 4% other. Mean knowledge scores for warfarin and INR self testing modules were 97.7%, median 100% (range 60-100%), and 98.1%, median 100% (range 60-100%). Measure of calm was high at 77.5% and worry was low at 39.5%, with respective medians being 30 (range 17-40) and 14 (range 10-32). HCP time per patient for warfarin and self-testing pre-intervention was 75 and 60 min, respectively. Using eKITE, HCP teaching time decreased to 15 and 14.7 min for warfarin and self-testing, respectively. Mean time in therapeutic range was (n=72 (on warfarin > 45 days)) 79% SD 15.2, Median 78.2 demonstrating outstanding INR control. Overall, patient preference for eKITE over usual teaching styles was high. eKITE is an effective teaching modality with the potential to revolutionize warfarin patient education from a two-dimensional tedious task to an engaging, low stress experience while reducing costs associated with HCP time resulting in safe and effective warfarin management.
