The article presents a case study of 57-year-old patient presented with drug-refractory atrial fibrillation and referred to hospital for pulmonary vein isolation (PVI). Topics discussed include revealing a large persistent foramen ovale in transesophageal echocardiography, using irrigated multielectrode ablation (IMEA) catheter for performing PVI, and feasibility of using an IMEA catheter and a 3D mapping system without using fluoroscopy for patients with persistent foramen ovale.
