The article discusses the case of a 75-year-old male patient who underwent transesophageal echocardiography (TEE) for a left atrial appendage thrombus evaluation prior to direct cardiac conversion. A real-time three-dimensional (3D) probe was used in performing the TEE. A ridge-like structure on the left atrial side of the atrial septum protruding into the left atrial lumen was detected by the 3D TEE. The overhung area did not show any thrombus or spontaneous echo contrast.
