The level of intact PTH in their controls corresponded to the level we measured in our controls, however the level of intact PTH in their patients on dialysis with high turnover was many times higher than the intact PTH we measured in our CKD patients. Not only does the level of PTH 1-84 increase with declining kidney function, but higher levels of different PTH fragments [[7]], and a prolonged half-life of the C-terminal PTH fragments has also been found in patients with CKD [[4]]. Hypercalcemia reduces overall PTH secretion, and it favors release of PTH fragments, including a possible 34- or 36 amino acid N-terminal peptide, a larger C-terminal peptide, and a PTH 7-84 fragment.
