The lengthening of core training within the same overall training time envelope of 7 years makes sense as < 40% of trainees progress to ST3 after only 2 years of core training [7]. Fifteen years later, another cohort of trainees failed to enter their preferred specialty training when another large change to training was made with the introduction of Modernising Medical Careers and run-through training [18]. Of the unsuccessful trainees surveyed, around 80% intended to seek "top-up" training in CT3 or non-training fellowship posts in order to meet the necessary entry requirements to compete for stage 2 posts available from 2023.
