This article discusses a case study involving a 15-year-old girl with metastatic melanoma. The girl initially showed intense uptake in lesions located in her breasts, lungs, bones, and retroauricular region on a 2-[18F]FDG PET/CT scan, leading to a diagnosis of metastatic breast carcinoma. However, a subsequent PET/CT scan with [68Ga]Ga-FAPI-46 showed high uptake only in the bones and retroauricular region. Further investigation revealed that the patient had a surgical removal of a mole from the preauricular region, which was not communicated to her family due to a language barrier. A second biopsy confirmed melanoma metastases in the breast lesions. The article suggests that incorporating [68Ga]Ga-FAPI-46 in addition to 2-[18F]FDG may help accurately characterize PET/CT lesions and reduce the risk of misinterpretation.
